No guarantees. • Pre-registered appointments will have applicable OFA forms completed for them in advance! Yes! This means you will not have to perform this tedious task or remember to bring your dog’s AKC and microchip info with you! Upon arrival at the Health Clinic check-in desk, please request your dog’s health clinic packet by last name of the owner, sign the enclosed OFA forms and make your dog available for microchip scanning for the staff to confirm accuracy and identity.
