THYROID SCREENING OFA Thyroid Database Screening Pre-registration: $175 Walk-in (if available): $200 Additional Information About This Test: Blood will be collected for this test. Please fast your dog at least 8 hours prior to your appointment time. The samples will be submitted to Michigan State Veterinary Endocrinology Lab, a certified lab by OFA. Shipping costs and material costs are included in the above cost of the test. Appointment Registration • A limited number of appointments will be available; pre-registration is required. • Time and schedule permitting, walk-in appointments may be available. No guarantees. • Pre-registered appointments will have applicable OFA forms completed for them in advance! Yes! This means you will not have to perform this tedious task or remember to bring your dog’s AKC and microchip info with you! Upon arrival at the Health Clinic check-in desk, please request your dog’s health clinic packet by last name of the owner, sign the enclosed OFA forms and make your dog available for microchip scanning for the staff to confirm accuracy and identity.
